Meet Dr. Azari

Surgical care doesn’t have to be overwhelming or confusing – Dr. Azari’s goal is to provide clear direction at each visit. He will thoroughly communicate with you to find the balance between your needs and what’s possible.
Dr. Azari is passionate about routine oral surgeries, such as removal of wisdom teeth and placement of dental implants, as well as complex reconstructive needs such as bone or tissue grafting, surgeries for orthodontics, TMJ surgeries, and jaw surgery for correction of malocclusion, breathing disorders, and facial proportions. Dr. Azari has spent over a decade as a trainee and as an attending surgeon on the cleft lip and palate team at the Shriners Hospital in Portland, OR.
Dr. Azari lives in Bend with his wife and two children, and like you, they love living in this outdoor wonderland. You can find them walking, biking, or swimming around town – proud to be part of this community!

Meet Dr. Welch

Dr. Welch brings calm and compassion to every surgery and consultation. He is dedicated to delivering safe and efficient care, and has expertise in routine oral surgeries such as wisdom tooth removal, impacted tooth removal, bone grafting and the insertion of dental implants. He is a safe and certified provider of IV anesthesia along with Dr. Schock and Dr. Azari.
Dr. Welch sought out Central Oregon for the trails and snow. When he’s not taking care of his patients, he is likely building airplanes or cultivating a bit of forest where he and his wife are raising their twin boys.

Meet Dr. Schlam
Dr. Mark Schlam was raised in Lexington Massachusetts, where he learned to love the outdoors in all seasons. His childhood was spent hiking, skiing, and exploring the mountains and coastline of New England. After high school, Dr. Schlam received his B.A. from Yale University. At Yale, he was a 4-year letterman on the varsity lacrosse team. During his undergraduate training, Dr. Schlam gained his EMT certification and valuable experience managing emergency situations.
Dr. Schlam obtained his dental degree from Tufts University School of Dental Medicine where he was awarded a Health Professions Scholarship from the United States Army. In dental school he served as an academic tutor to his peers, teaching assistant in Oral Surgery, and BLS instructor. At Tufts, Dr. Schlam was selected to participate on a mission trip, where he traveled to rural regions of the Dominican Republic and provided surgical services to the local communities.
After dental school, Dr. Schlam became an active duty Army officer, and completed the Oral and Maxillofacial Surgery residency training program at Womack Army Medical Center in Fort Bragg, NC. Dr. Schlam trained extensively in facial reconstruction and dental implant therapy, advanced bone grafting techniques, anesthesia, orthognathic surgery, cosmetic surgery, and trauma. He has special interest in complex full mouth dental rehabilitation and enjoys working closely with his restorative colleagues.
Dr. Schlam places patient safety and comfort as his top priorities and strives provide a positive experience for all of his patients.
In his free time, Dr. Schlam enjoys spending time with his wife and two young children, and finding fun places to mountain bike, surf, race triathlons and snowboard.
